Name Badges

All new members will receive a permanent name badge. There is a choice of full size (3 inches wide and 2 inches tall) or “Mini” (3 inches wide and 1.25 tall). Images are below (not to scale). They also come with pin or magnet fastening.   When completed, badges will be left for the member in the “Name Badges” box at the monitor station at the pavilion.

Order Replacement Name Badge

If you would like to order a new laser-engraved name badge with a magnetic or pin fastening, complete the form below.

Cost of a badge is $8.00.

Complete and submit the form below online. Do not submit paper forms.

Put a check for $8 payable to Grand Pickleball Club in an envelope and put it in the big black Dropbox Mailbox on the bottom shelf across from the island in the pavilion (see the picture below).

When your payment has been received and your new badge is ready, the new badge will be put in the Pickleball Club Name Badge box (on the top shelf across from the island in the pavilion).

Please check the name badge box when you are at the courts.
